Creating a logit model to estimate participation?

Hello, I am doing research on the impact of certain household and child variables, such as household assets (X1), access to sanitation facilities (X2), religion of household head (X3), child birth weight (X4), on health outcome (Y) -which is the dependent variable.. I want to do logistic regression. how do I write up a logit model for estimating the probability of participation? basically, what formula/equation do i use in order to form a logit model to estimate the probability of participation. My goal is to create a logit model with the variables above, and then estimate the propensity score through logistic regression and on to do propensity score matching. For propensity matching, I am using a treatment group defined as "households that have access to piped water" and a control group defined as "households that have no access to piped water" The outcome variable is child health outcome, which is a binary variable (ie. child has fever or not)
